Information submitted through our website contact form is used solely to respond to your inquiry. For patients under 18, parents or legal guardians generally have the right to access their child's health information. Exceptions may apply for certain sensitive services as permitted by Georgia law. Patients who reach age 18 must provide their own authorization for release of records.
